Prospective Clients
I will need a referral from a physician to see you. This is a necessary
step for voice patients, and for patients with speech sound disorders or
communication problems, this will facilitate my understanding of your
problem. It is also an important step for your obtaining insurance
reimbursement for my services.
If you or your child has a voice disorder, please see an
otolaryngologist for a laryngeal examination. This physician can then
fax me his/her results, using the form below. You can also print this information
out for your physician and take it to your examination.
If you or your child has a speech sound articulation disorder, resonance
disorder, or other problem with communication, please see an
otolaryngologist, pediatrician (for a child) or family or other
physician for an examination. This physician can then fax me his/her
results, using the form below. You can also print this information out for your
physician and take it to your examination.
